I'm pretty sure that your Magtek reader will need to be reprogrammed. Please call tech support for assistance on that. If we can't reprogram over the phone, they'll send you an RMA.

Could be that, that particular check is not reading correcly or you may also try pressing the caps lock key before swiping the check.

Did you try the cap log suggestion?

After that, I think we're looking at an RMA or tech support.
